Write entries people can respond to
Describe the issue in the client’s language before adding technical detail. Explain the affected service, the potential impact and the evidence supporting the finding. Make uncertainty visible.
Invite feedback at the point of the risk
Clients may know about a planned replacement, an operational dependency or a business deadline that changes the proposed response. Keep that feedback with the risk so both teams can revisit the reasoning.
- What business context are we missing?
- Does the proposed priority reflect your operations?
- Is a related project already planned?
- Who should help agree the next action?
Keep feedback and decisions distinct
A comment is useful context; it is not automatically a decision or risk acceptance. Record the agreed outcome separately, including the accountable person and next review date.
When new evidence changes the assessment, explain what changed and bring the entry back into the client conversation.
